Your journey starts here! Complete the Evaluation Registration Form to secure your spot. Be sure to have the following documents ready to upload:
✅ Current athlete photo
✅ Filled-out evaluation packet (download from above link)
✅ Copy of athlete’s birth certificate
✅ Copy of parent/guardian ID
✅ Copy of card on file (for autopay verification)
✅ Copy of insurance card
📌 Important: All new athletes must also complete the Gym Waiver Registration Form (also linked inside the form) to create an account and sign required documents. This step is required but separate from evaluation registration—be sure to submit both!
